Summary:
|
William Longer, Board Attorney, will ask the Board to consider a services agreement with the Community Care Network, Inc. (CCNI) for services provided by the Brickie Clinic and Immediate Care at Hobart High School. Mr. Longer has reviewed the services agreement. The term of the agreement is two years, January 1, 2022, through December 31, 2023, with an option for an automatic renewal for one additional year. In addition, school district employees and their dependents who are covered under the school district's health insurance program will have access to receive services at Valparaiso Health Center Immediate Care, 3800 St. Mary Drive, Valparaiso and at the South Valpo Immediate Care, 1451 W. Morthland Drive, Valparaiso, Indiana.
Another bonus about the Brickie Clinic and Immediate Care is that it is also open to provide services our students, parents, and community members and is a wonderful opportunity and addition of services for School City of Hobart patrons.
